Kamp Mill Waiver Form

Dear Kamp Mill Participants,

We are pleased that you will be joining Dr. David Bernstein for the Kamp Mill Heritage Seminar. Please read the below information carefully. Participant registration will be considered incomplete without signature to terms and conditions.

Payment:

  • $500 deposit must be submitted by February 15th, 2018. Please note: Heritage cannot guarantee reservations submitted after this date.
  • Remaining balance must be submitted by April 15, 2018.

Cancellation Policy:

  • $300 non-refundable registration fee per paying participant
  •  100 to 61 days prior to departure – 50% of the total transaction non-refundable.
  • 60-15 days prior to departure – 75% of the total transaction non-refundable.
  • 14 days prior to departure – 100% of the total transaction non-refundable.

Passports:

Passports must be valid at least 6 months before departure.  It is recommended to bring a copy of one’s passport on the Seminar.

Insurance:

It is the responsibility of participants to provide his/her own medical and travel insurance.

Fee Does Not Include:

Expenses of a personal nature, including drinks, taxis, phone calls etc.

Tips for service providers – According to discretion of participants.
